Be aware this form it is an application for enrolment. 

Each individual enrolment is checked and accessed for suitability by our Head Trainer. Not all dogs or all situations are suited to group training lessons and to ensure a positive learning experience and great equal learning opportunities for all we need to ensure the right mix of dogs is put into each class. 

Please complete this form as honestly as you can to ensure the best possible chance of securing one of our limited spots.

    Dogs Vaccinations

    Please by aware your application for enrolment will not be accepted for processing without current up-to-date vaccination information.
    You will also need to submit evidence verifying your vaccination information.
    Please submit a current vaccination certificate (including Bordetella for kennel cough).
    Vaccination cards need to be scanned or photographed and attached here with your enrolment form, without them your application can not proceed.
